Completing this form does not guarantee placement on a team. All players will be contacted by a coach by 06/20/08.  If numbers require tryouts, they will be held on 06/23/09 and 06/24/09. Please email any questions to coachmikeclark@gmail.com
Players are placed on teams at the discretion of CSA. Younger players may play on older teams but at no time may an older player play on a younger team. It is preferred that whenever possible a player plays with his/her grade. 
 
Important – U11 thru U14 teams are formed to participate in both the Fall and Spring of each year. You still need to return this form for consideration to be added on a team for spring only. There is no guarantee a team will have space to add players in the spring.
